Simulated scenario

The "Loss of Trust"

A production facility experienced an 82% revenue drop in one quarter. Internal audits reported "normality" while the shop floor was bleeding money. Management reported 98% OEE.
What S2R found After a heater replacement, maintenance connected the resistor in STAR instead of DELTA — causing 33% loss of thermal power. Operators bypassed the PLC to manual mode to hide "Low Temp" alarms that penalized their KPIs. The real sealing temperature was 160°C vs 180°C nominal. Result: 800,000 DPMO.
Corrective action Reconnection to Delta and PLC restoration. Time: 20 minutes. Cost: €0. Plus a 5-step protocol for alarm escalation and visual fail-safe on the sealing unit.
